I made a few modifications, I believe these changes will make it easier for both of us and the students as we will eliminate the opt-in process. However, I can still have the students respond with their completed forms if necessary. Let me know.
- Students are forced to use their student email, if they do not enter their student email the form won’t be delivered.
- Students will receive an email notifying them that a form has been submitted with their email account, they are instructed to contact the college immediately if they were not the one who submitted the form.
- I added a signature box that captures an image of the signature.
- PII (Personally Identified Information) is encrypted and any identifying documentation uploaded is not stored on the server.
- Form results come in a PDF format.
